Essay

The research is devoted to improvement of anaesthesia for major oncoorthopedic surgery by means of examination of efficiency of fluid therapy regimens, and hemodynamic monitoring optimization, and choosing of intraoperative hemodynamic correction method for decreasing of postoperative morbidity. We analyzed the results of using of restrictive regimen of fluid therapy and Goal-Directed fluid therapy in 70 patients in two equal groups. We found a rationale for using of Goal-Directed fluid therapy for intraoperative hemodynamic stabilization in patients with major orthopedic surgery for lower limbs bone oncology. Due to complex comparative studies of central and peripheral hemodynamics, and lactate production we obtained data that allow us to recommend Goal-Directed correction of cardiac output and vascular tone by means of infusion of colloid solutions. We found correlation between cardiac output, vascular resistance, capillary perfusion, lactate production, and postoperative complications.
