The dissertation is devoted to solving the problem of improving the results of surgical treatment of patients with bulky liver formations by identifying features of liver angioarhitectonics and individualizing surgical procedures to minimize postoperative biliary complications. Comparative analysis of the results of complex diagnosis and intraoperative data showed high accuracy of multi-spinal computed tomography in the initial assessment of angioarchitectonics of the liver: up to 97.0% - the arterial bed, up to 96.0% - the anatomy of the portal vein, up to 90.0% - the structure of the venous outflow . The leading methods for diagnosing the causes and the source of bile from liver resection are ultrasound, video laparoscopy, and endoscopic retrograde pancreatic cholangiography. The application of the developed tactical approach to the choice of the method of surgical treatment of bulky liver formations, taking into account the individual features of the hepatic architectonics, provided a reduction in the number of biliary complications from 29.3% in the comparison group, to 9.6% in the main group, which also reduced the total number of complications from 36.7% to 20.9%, respectively, and reduce mortality from 7.3% to 4.8%.
