Essay

The reseasrch presents the results of surgical treatment of patients with acute adhesive obstruction of small gut by optimizing the diagnosis of the stage of the disease, studying the level of intra-abdominal pressure and on these grounds determining of indications for the choice of the method of surgical treatment and post-operative period. It was established that acute adhesive obstruction of small gut at the stage of compensation is accompanied by the development of the intra-abdominal hypertension of the 1st degree in 73.9% of the patients, and intra-peritoneal hypertension of the 2nd degree in 79.8% of the patients is at the stage of decompensation, which has the highest sensitivity and diagnostic accuracy at these stages, respectively. The developed algorithm of surgical tactics in patients with acute adhesive obstruction of small gut taking into account intra-abdominal pressure has allowed to optimize the diagnosis of the stage of the disease, indications for the choice of surgical treatment in combination with the application of the proposed methods of postoperative treatment of such patients, which ensured decreasing the frequency of purulent and septic complications by 5.9% and the level of postoperative lethality up to 5.8%.
