The prospective study included 265 patients with varicose veins of the lower extremities, that underwent combined EVLT with application of bare-tip and radial fibers with following comparable analysis of treatment results (frequency of the traumatic neuritis of saphenous nerve, recurrence of the vertical reflux, scale of severity of venous dysfunction according to VSS, quality of life according to CIVIQ-2) within a year after surgery. There was established a dependence of the results of treatment on the diameter of GSV. The biggest critical diameter of GSV, in case of which the probability of recanalization overcomes 50% accounts over 16 mm when bare-tip fiber is applied, and 23 mm in case of radial fiber usage. There was created an algorithm of treatment of patients with varicose veins of the lower extremities with differential application of different types of fibers, improved combined methodology of elimination of continuous vertical reflux and crossectomy allowed accurately to decrease the frequency of post-traumatic neuritis in a year-long post-operative period (in patients with total reflux) from 35,5% to 0%, the frequency of recurrence of the vertical reflux from 11,5% to 1,8%, the average values of the CAD score from 2,8 to 1,8, increase global index of life quality from 88,2 to 92,9, provide increased percentage of good treatment results - 92,9% vs. 81,8%.
