Balinska M. Grounding of comprehensive treatment of old and senile age with chronic venous insufficiency of the lower limbs in stage of venous ulcers

The dissertation is focused on research of laboratory and pathomorphological aspects of chronic insufficiency (CVI) in stage of leg venous ulcers and treatment this disease of old and senile age. Among causes of CVI are violation of venous outflow and dysfunction of endothelium. Vascular endothelial growth factor (VEGF-А), nitric oxide (NO), increasing of blood level of nitrites and index of induction of apoptosis in endothelial cells play a significant role in development of endothelial dysfunction. The treatment included conservative and surgical treatment. Conservative treatment consisted of systemic therapy: antibiotics, vascular drugs, anticoagulants, antiaggregants. We introduced a comprehensive using of nitric oxide by "Plason" apparatus and a application antimicrobial sorbent, which includes aerosil and serratiopeptidase as a local therapy. Using of this methods leaded to reduce of pain by 3,9±0,5 of treatment, cleaning of venous ulcer by 4,7±1,1 day, appearance of granulation by 4,4±1,1 and edge epithelization by 4,9±0,9. Ulcer became healed by 14,2±2,3 day. Comprehensive using of this methods of treatment of old and senile age with chronic venous insufficiency of the lower limbs in stage of venous ulcers increases effectiveness of treatment, accelerate healing of ulcers and improves the quality of life of patients.
