Essay

Object – multiple pregnancy in women at risk for preterm labor; goal – reduce the incidence of preterm birth in women with multiple pregnancies based on the development and implementation of prognostic and therapeutic and preventive measures; methods – anamnestic, clinical, laboratory, biochemical, instrumental, statistical, mathematical; results – clarified scientific data on risk factors for preterm birth in women with multiple pregnancies, as evidenced by a significant predominance of preterm births, bacterial vaginosis, urinary tract infections, manifested by clinically asymptomatic bacteriuria, threatened miscarriage, shortening of the cervix ≤ 25 mm; the idea of the possibilities of stratification of risk groups and prediction of preterm labor and their timing in women with multiple pregnancies was expanded, evidenced by: a sevenfold increase in the probability of preterm birth at the threshold value of the integrative history index developed by us, initial health status, pregnancy ≥ -2, 14 scores (p <0.001) and a tenfold increase in the probability of preterm birth within the next 7 days, with a comprehensive definition of integrative and Dex anamnesis hundred output health, pregnancy ≥ -2,14 points and the concentration of IL-6 in the cervical mucus ≥ 41,65 pg / ml (p = 0,013); supplemented scientific data on the dependence of obstetric and perinatal outcomes of multiple pregnancies on the degree of shortening of the cervix (<20 mm or ≥20 mm), provides a differentiated approach to the choice of prevention strategies for preterm birth depending on the results of cervicometry; branch – medicine.
