Essay

The dissertation is devoted to the improvement of the results of surgical treatment of patients with complications of acute pancreatitis with the use of minimally invasive technologies. The analysis of the results of treatment of 352 patients with acute pancreatitis during the period from 2007 to 2017, received complex treatment in the clinic of the Department of Surgery and Proctology NMAPE named P. L. Shupyk. Age of patients: from 17 to 79 years. The average age is 46.2±2.1 years. Men were 108 (61 %), women - 69 (39 %). In the first (main) group of 177 patients, treatment was performed according to the developed algorithm using the minimally invasive techniques for the period of 2011-2017. The comparison group consisted of 175 patients who underwent the traditional treatment of acute pancreatitis and its complications from 2007 to 2011. The results of the treatment of patients in the comparison group were evaluated retrospectively based on the history of the disease. A diagnostic and treatment algorithm has been introduced for patients with severe acute pancreatitis, taking into account the phase nature of the course of the disease, the use of minimally invasive technologies and the individual approach has made it possible: reduce the patient's stay in the hospital for 4 days, relative to the comparison group, with a better quality of life for patients; to increase the number of patients treated with conservative methods by 6.5 % with a positive result; reduce the number of patients who performed open surgical interventions 4 times, with 56.57 % in the comparison group to 13.56 % in the main group. to reduce the overall mortality rate with acute pancreatitis from 17.7 % in the comparison group to 4.52 % in the main group.
