Essay

The thesis is devoted to the experimental and clinical substantiation of the use of fibrin glue and temporary intestinal endoprotectors in intestinal surgery. The aim of the thesis is to improve the results of the formation of intestinal anastomoses by justifying and experimenting with the development of reliable methods for their implementation and protection by temporary intestinal antimicrobial endoprotectors from PVA, as well as introduction into clinical practice. The dissertation work consists of two parts - experimental, which was performed on 162 laboratory animals in the form of five series of experiments; and clinical, including 28 patients, who in detached and urgent order in surgical departments No. 1 and No. 2 of Kiev City Clinical Hospital № 6 performed surgical interventions on different parts of the colon with the use of the developed method of formation and protection of intestinal anastomoses. Based on the results of hydropressions, bacteriological and morphological methods of research, the advantages of double protection of colonic anastomoses are proved - sealed from the serous membrane by fibrinous autoclave and additionally by intraintestinal injection into the anastomosis area of the temporary intestinal antimicrobial endoprotector from PVA, which was manifested by the lack of microflora growth at the time of 24, 48 and 72 hours after the operation. The introduction of experimental development of the formation and protection of large intestine anastomoses in the clinic in 28 patients both in detached and urgent order confirmed the experimental results obtained in the experiment.
