Scientific research is devoted to the improvement of diagnostics of coronary heart disease destabilization on the basis of determine markers of non-specific systemic inflammation, endothelial dysfunction and endocrine function of adipose tissue, and optimization of treatment of patients with coronary heart diseases. The obtained data confirmed the role of non-specific inflammation, endothelial dysfunction and violation of the endocrine function of adipose tissue in the progression of the atherosclerotic process in coronary heart disease patients. More significant changes of these indicators were observed in patients with unstable course of the disease. The prognostic significance of biochemical indicators, which reflect the main links in the pathogenesis of the disease, was evaluated based on the results of a two-year observation. There was a clear link between the development of adverse cardiovascular events and biomarkers of inflammation (levels of hsCRP, TNF-?, PAPP-A) and endothelial dysfunction (sVCAM and ET-1 levels), and slightly lower - with leptin level. It was established that inclusion in a complex optimal drug therapy, designed according to modern recommendations, a fixed combination of meldonium and ?-buterobetaine contributed to both a more pronounced improvement in the life quality of coronary heart disease patients and a significant improvement in the parameters of vasoregulate and adhesive functions of the endothelium.
