105 patients with a thermal trauma were inspected and analyzed in this work. The study was determined that the presence of concomitant pathology, the etiologic factor, with simultaneous respiratory tract damage probably influenced the increase of the preoperative intensive care terms. The first time it was substantiated that the risk of the complicated flow of burn disease is probably associated with the area of deep burns (р=0,001), volume of infusion therapy on the 1st day (р=0,001) and the 2nd day (p=0,005) burn shock as well as an increase in the required volume of infusion therapy to achieve the target rate of diuresis > 0.5 ml / kg / h (p=0,001). A comparative characteristc of the dynamics perioperative indicators of metabolic, inflammatory response, multiple organ dysfunction was conducted. In determining the optimal timing of preoperative preparation in patients with severe and extremely severe burns, it is justified to use the principles of "Damage Control" tactics, which is aimed at the body's abiliting of adaptation. It coused the development of the scheme perioperative management of patients with the severe burns, contributing to the recovery of homeostasis. on the background of surgical treatment.
