Feskov V. Tactics of infertility treatment using auxiliary reproductive technologies in women with ovarian endometriosis

The thesis deals with the quality improvement of provision of obstetric and gynecological care for women with ovarian endometriosis in order to increase the effectiveness of the application of auxiliary reproductive technologies by improving infertility treatment. To achieve the goal, the definition and evaluation of hormonal, cytokine and endothelial status, paracrine factors, vitamin D content in 90 women with endometriosis of the ovaries who received infertility treatment with assisted reproductive technologies (ART) and 30 women of the control group were evaluated. As a result of the multicomponent clinical and laboratory examination of women, a modern algorithm for complex infertility treatment using ART methods for women with ovarian endometriosis with a surgical intervention plan has been developed. The conducted research will improve the reproductive health of patients with ovarian endometriosis by preventing the onset of a major problem: the premature termination of the reproductive function. Based on the definition of paracrine regulation violations of folliculogenesis in women of reproductive age, the diagnosis was improved and individual pathogenesis chains of ovarian reserve reduction in women after surgical treatment of ovarian endometriosis were defined. Thanks to the developed modern algorithm of the extracorporal fertilization program, it is planned to achieve a decrease in the frequency of ovarian reserve losses, which will enable women to implement the reproductive function.
