Clinical, biochemical and neurohumoral features of patients with resistant arterial hypertension (RAH) were studied. A comparative analysis of the response to treatment with spironolactone, eplerenone, nebivolol and moxonidinе showed that the most effective 4th component of therapy for these patients are spironolactone and eplerenone. Based on the comprehensive evaluation of basal biochemical parameters, activities of low-grade inflammation and neurohumoral system of blood pressure regulation the predictors of efficacy of the 4th medicine for treatment of RAH patients were determined. Approaches to diagnosis of pseudoresistant and true RAH were improved.
