Essay

The aim of the thesis is to increase the effectiveness of the results of surgical treatment of ventral and postoperative ventral hernias by developing and introducing new laparoscopic hernioplastic techniques and a new mesh implant. The work is based on studying results of surgical treatment of 277 patients with ventral hernias (umbilical, paraumbilical, and hernias of the white line of the abdomen) and postoperative ventral hernias. In the comparison group (n = 93), open hernia repair was performed using the "Onlay" method. In the main group (n = 184), 91 patients underwent classical laparoscopic hernia repair, and 93 patients underwent improved methods of laparoscopic hernia repair: a developed method for performing laparoscopic hernioplasty of ventral hernias (Patent of Ukraine No. 62541), a developed technique of combined laparoscopic hernioplasty of ventral hernias and the method of laparoscopic hernioplasty of ventral hernias using the developed mesh implant (Patent of Ukraine No. 70545). The use of advanced methods of laparoscopic hernioplasty can significantly reduce the number of intraoperative and postoperative complications. The probability of intestinal damage during surgery significantly decreases, the duration of operations decreases from (138.4±8.5) to (63.7±9.7) minutes (p<0.05), the frequency of seromа decreases from 8.7 to 4.38 %, infection of postoperative wounds from 2.6 to 1.0 %, as well as the frequency of hernia recurrences from 14.8 to 2.5 % (p<0.05).
