Essay

Object of study: essential arterial hypertension osteodeficiency, of the 150 patiients. In the thesis research the solution of the scientific problem of general practice - family medicine, namely, increase of the efficiency of treatment of patients with essential hypertension (AH) by means of determination and scientific substantiation of informative value of various methods of blood pressure monitoring (BP), determination of the most informative method of home blood pressure monitoring( НBPM), the indicators of which are most close to the average daily pressure during daytime blood pressure monitoring, and the use of this particular methods of frequent self-measurement of blood pressure every 2-3 hours, in assessing the sensitivity of antihypertensive therapy in the practice of family doctors, is given. Methods of research: clinical-anamnestic - Research methods: general clinical examination of patients (analysis of complaints, analysis of history and objective examination to assess the clinical manifestations of hypertension), instrumental (echocardiography to determine structural changes in the heart, determination of systolic and diastolic functions of the left ventricle electrocardiography (ECG) to determine the heart rate, determination of signs of left ventricular hypertrophy, determination of changes in repolarization processes, DMAD for determining the level of blood pressure in the home, AMAD for determining daily blood pressure profile, office measurement of blood pressure to determine the level of blood pressure in the doctor's office),). laboratory tests treatment adherence, results of treatment, questionnaires for assessing the quality of life). The scientific novelty of the results obtained is determination of informative value of different in frequency methods of measurement of (HBPM), compared to the indicators of daily and office blood pressure. The role of home monitoring in improving the effectiveness of treatment for patients with hypertension based on the application of HBPM techniques in 2-3 hours, to determine the sensitivity to antihypertensive drugs of baseline therapy, on the example of liрrazid, in the practice of family physician. In the dissertation research, the informativeness of different depending on the frequency of blood pressure measurements, HBPM techniques in comparison with the indicators of ambulatory blood pressure monitoring (ABPM) and office blood pressure measurements was analyzed. It is proved that the most informative method is HBPM with a frequency of blood pressure measurements every 2-3 hours during the day, at which the average daily indices of systolic blood pressure (SBP) are almost similar to those of the average daily SBP obtained from ABPM. Determination of individual sensitivity of a patient to antihypertensive drug (AHD), before appointment of treatment, allows to optimize complex antihypertensive therapy (AHT), to reach the target level of blood pressure and increase the adherence to treatment of patients in the practice of a family doctor. In the thesis research the solution of the scientific problem of general practice - family medicine, namely, increase of the efficiency of treatment of patients with essential hypertension (AH) in the practice of a family doctor by means of determination of the most informative by frequency method of home blood pressure monitoring and assessing the individual sensitivity of patients to baseline antihypertensive therapy, is given.
