During our clinical trial we made the complex analysis of lipid profile, markers of nephropathy, endothelial dysfunction, structural and functional exponents, uricemia in patients with arterial hypertension and type 2 diabetes mellitus for improvement the diagnostic and treatment of such coexistent pathology. The determination of albumin to creatinine ratio in the single morning urine sample is recommended for the nephropathy diagnostic in patients with arterial hypertension and type 2 diabetes mellitus. The calculation of left ventricular mass index for patients with such coexistent pathology as a left ventricular mass to height in the degree of 2,7 ratio would help to optimize the detection and avoidance of hypodiagnostics of left ventricular hypertrophy. Endothelin-1 is the indicator of endothelial function. Tumor necrosis factor-? is the important marker of immune and inflammatory disorders. The soluble vascular cell adhesion molecule is common marker of endothelial dysfunction and inflammatory disorders. We developed a new approach of telmisartan doses definition. The prescription of such daily dose of telmisartan as 40 mg is recommended if the level of endothelin-1 is less than 10 pg / ml and as 80 mg of corresponding drug, if the level of endothelin-1 is greater than 10 pg / ml in patients with type 2 diabetes mellitus and arterial hypertension for organoprotective action
