The dissertation is devoted to prognosis of adverse course and increasing of treatment efficiency in patients with acute ST-segment elevation myocardial infarction. Multifactorial prognostic models have been developed, based on the determination of level of serum soluble ST2 level in the first day of the disease and other clinical biomarkers in patients with acute ST-segment elevation myocardial infarction that allows to predict unfavorable course of acute myocardial infarction during 6-months period. The value of ST2 level in predicting of development of progressive renal function decreasing was evaluated and the importance of this biomarker determining in the first 24 hours of the disease in patients with acute ST-segment elevation myocardial infarction was proved. The effect of ST2 on left ventricular myocardial remodeling was studied in patients with acute ST-segment elevation myocardial infarction after revascularization. Determination of ST2 allows to predict the development of left ventricular remodeling in 6 months of observation. Advisability of prescription of high doses of bisoprolol (5 mg or more) for high-risk patients was justified scientifically to prevent the development of adverse events. It has been proved that prescription of eplerenone to patients with acute myocardial infarction with high ST level despite of left ventricular ejection fraction prevents development of pathological left ventricular remodeling and improves the course of the disease.
