The object is the course of the early period of traumatic illness with a combined trauma of the abdominal cavity, complicated by hypovolemic shock in the background of ischemia-reperfusion of the limb; The purpose - to clarify the role of ischemia-reperfusion of the limb in the pathogenesis of structural and functional disorders of the kidneys under conditions of massive blood loss with combined trauma of the abdominal cavity organs; methods - bibosemantic; graphic; experimental; functional; biochemical; morphological; mathematical and statistical; results - the peculiarities of the pathogenesis of morpho-functional disorders of the kidneys in the period of early manifestations of traumatic illness after ischemic-reperfusion syndrome of the limbs and its combination with the closed trauma of the abdominal cavity, complicated by hypovolemic shock; It has been shown that ischemia-reperfusion of extremities in combination with trauma of the abdominal cavity complicated by hypovolemic shock affects the functional state of the kidneys, which manifests itself by a significant decrease in the velocity of glomerular filtration and diuresis, slowing proximal and distal transport of sodium ions, increasing the concentration of creatinine in urine, decreasing its excretion and clearance; It was revealed that ischemic-reperfusion syndrome causes the growth of TBK-active products of LPO and diene conjugates in the kidney tissue; found that the level of these indicators increases compared with the control and reaches the maximum values ​​after 7 days post-traumatic period; the analysis of morphological changes in the tissues of the kidney under the conditions of modeling ischemic-reperfusion limb syndrome and its combination with the trauma of the abdominal cavity complicated by hypovolemic shock; It was found that morphological changes reach the maximum values ​​after 7 days post-traumatic period. Significant changes are marked dystrophic and necrotic changes of the epithelial cells of the excretory tubules.
