Essay

The dissertation is devoted to the optimization of diagnosis and treatment of chronic heart failure (CHF) with atrial fibrillation (AF) and renal dysfunction on the basis of determining the state of cardiohemodynamics, endothelial function, markers of glomerular and tubular damage, level of galectin-3 and glycation end-products. According to the results of the study high level of galectine-3, β-2 microglobuline, albumin, end-glycation products were associated with renal function decrease in patients with heart failure and atrial fibrillation. Functional state of kidneys in patients with CHF and AF was strongly connected with BMI, LA size and end-glycation products level. Was established prognostic value of severe albuminuria and high level of end-glycation products as additional markers of poor prognosis for development of renal failure in patients with heart failure and atrial fibrillation. Was defined that development of albuminuria may be influenced by level of β-2 microglobuline and LA size. It has been demonstrated that the use of irbesartan in patients with CHF and AF in terms of the correction of BP contributed to the reduction of albuminuria and the progression of renal failure.
