Essay

Object – orthopedic complications of acute hematogenous osteomyelitis in children; goal – improving the results of early diagnosis, prevention and treatment of orthopedic complications of acute hematogenous osteomyelitis in children based on the study of changes in mineral metabolism in various forms of acute hematogenous osteomyelitis using laboratory and instrumental tools and developing optimal methods for their correction; results – the clinical prevalence of orthopedic complications was determined, which was maximum after septic and toxicoseptic forms of acute hematogenous osteomyelitis, with an uneven quantitative distribution by age; it was proved that in newborns within 1-2 months after acute hematogenous osteomyelitis in 40.0 ± 12.65% early orthopedic complications in the form of pathological dislocations of the femur without destruction; scientific information has been supplemented that the main mechanism for the occurrence of osteopenic syndrome in children with acute hematogenous osteomyelitis is a decrease in the functional activity of osteoblasts and bone formation processes, which should be taken into account when prescribing drug therapy; the role and patterns of mineral metabolic disorders in children of different age groups with orthopedic complications after acute hematogenous osteomyelitis during the entire period of follow-up observation; new up-to-date data on the study of the transport form of vitamin D3 (25OHD3) were obtained, laboratory markers of bone mineral metabolism were systematized for the early diagnosis and prevention of orthopedic complications of acute hematogenous osteomyelitis; based on the study of densitometric indicators, the pattern of development of orthopedic complications of acute hematogenous osteomyelitis in children was established, which in 56.25 ± 7.16% revealed violations of bone architectonics and the delay of endochondral ossification from passport ossification by 3-5 years; a complex scheme of early diagnosis, treatment and prevention of orthopedic complications of acute hematogenous osteomyelitis is proposed taking into account periods of increased growth of children in certain age groups; branch – medicine.
