The dissertation is devoted to the solution of the actual problem of modern coloproctology - improvement of the results of surgical treatment, quality of life, social and labor rehabilitation of patients with non-tumor diseases of the colon: ulcerative colitis, Crohn's disease, and related adenomatous polyphthisis. resection of the rectum mucosectomy of the surgical anal canal in the short small intestine due to forced removal tion of the terminal area, a short mesentery of the small intestine, loose type architectonics superior mesenteric artery and functional disorders as "irritable syndrome of the small intestine." The essence of the thesis is to develop new reconstructive operations, which were used in 32 patients of the main group. Among them, the method of dislocation of the small intestine graft in the pelvic cavity was used-in 15 (46.8 %) patients, the method of formation of the pelvic small intestine reservoir - in 14 (43.7%), ileoendoanal anastomosis - in 32 (100 %). The comparison group consisted of 36 patients. They underwent the accepted standard of reconstructive surgery. A comparative analysis of the results of the use of new reconstructive operations in patients of the main group with the results of traditional operations in patients of the comparison group was carried out. The advantages of new reconstructive operations are shown - expanding the indications for their use-in 11 (34.4 %) patients, reducing postoperative complications from 27.7 to 9.3 % (р<0,05), increasing the number of patients with favorable functional results from 42.7 to 93.3 %, reducing the incidence of severe forms of postcolectomy, diarrhoeal syndromes from 41.6 to 13.3 % (р<0,05), secondary anal incontinence syndrome from 35.5 to 6.7 % (р<0,05), increasing the number of patients with favorable indicators of quality of life and socio - labor rehabilitation from 63.8 to 90.6 % (р<0,05).
