The dissertation is devoted to increasing the effect of treatment of purulent soft tissue wounds for HIV-infected patients using the developed method of complex surgical treatment with ultrasonic cavitation and VAC-therapy. An analysis of the treatment of 271 HIV-infected patients with purulent soft tissue wounds was performed. Age of patients from 18 to 45 years. Depending on the method of treatment of purulent soft tissue wounds, patients of the main group were divided into four subgroups: Subgroup I (90 HIV-infected patients) - treatment of purulent wounds was performed by traditional methods: opening of the purulentlesion, necrectomy, wound healing with 0.02 % decamethoxine solution, wound drainage, antibacterial therapy according to the sensitivity of microorganisms, detoxification; Subgroup II (59 HIV-infected patients) - the traditional method of treatment of purulent wounds was supplemented by ultrasonic cavitation of wounds with lavage of 0.02 % decamethoxine solution; Subgroup III (62 HIV-infected patients) - traditional methods of treatment of purulent wounds were supplemented by VAC-therapy; Subgroup IV (60 HIV-infected patients) - traditional treatment of purulent wounds was supplemented by ultrasonic cavitation with lavage of 0.02 % decamethoxine solution and VAC therapy. There was proven the effectiveness of ultrasonic cavitation and VAC-therapy in complex surgical treatment in comparison with traditional methods, namely it reduces the regression of signs of inflammation: redness from 12.0±0.3 to 4.3±0.1 days; perifocal edema from 12.4±0.3 to 5.2±0.1 days, wound cleansing from necrotic tissue from 13.1±0.4 to 5.1±0.1 days, the appearance of granulations 7.2±0.3 vs. 15.7±0.5 days, wound healing was achieved 10.2±0.5 vs. 21.4±0.8 days.
