The dissertation focuses on establishing the features of lung damage after ischemia-reperfusion of the limb, acute blood loss and their combination after 1, 2 hours, 1, 7 and 14 days of the reperfusion period as well as highlighthig the efficacy of carbacetam in reducing the manifestations of the detected disorders. In the simulation of acute blood loss complicated by ischemia-reperfusion of the limb, there is a layering of pathogenic influence of each of the effects, in particular, which is manifested by increased apoptosis in the bronchoalveolar lavage leukocytes, enhanced generation of active forms of oxygen and reduction of transmembrane potential as well as a decrease in succinate dehydrogenase and cytochrome oxidase activities in pulmonary leukocytes. There is a greater increase in the content of lipid hydroperoxides and thiobarbituric acid reactive substances (TBARS) in the lungs, a decrease in the content of reduced glutathione as well as in the activities of superoxide dismutase, catalase, glutathione peroxidase and glutathione reductase. Most of the detected disorders are characterized by two maxima: the first – after 1 day of the experiment and the second with less intensity – after 14 days. In the structural elements of the lungs increase the manifestations of emphysematous dilation of the parenchyma, uneven blood supply of the vessels with a predominance of tissue ischemia, thinning of the interalveolar septa with areas of rupture and the formation of cellular infiltrates in the stroma. The administration of carbacetam in rats with acute blood loss complicated by ischemia-reperfusion of the limb is accompanied by decreased functional, biochemical and morphological manifestations of acute lung injury, which indicates its availability and requires further study.
