The thesis is devoted to improving the effectiveness of treatment of arterial hypertension (AH) and determining its prognostic impact in patients after myocardial infarction (MI) based on the study of its frequency and medical and socio-economic factors that determine the quality of AH control. A significant frequency of hypertension in patients in the remote period after MI has been established. It has been proven that the control of AH is insufficient - in the vast majority of patients blood pressure (BP) exceeds the recommended target values. The main medical and social factors that determine the low quality of treatment of AH have been identified. During long-term follow-up, it was proved that the prognostic value is not primarily the diagnosis of AH, but the effectiveness of its control and the quality of medical observation with the achievement of target BP values. It was found that in some patients with hypertension after MI during ambulatory blood pressure monitoring long episodes of excessive BP are registered; factors that increase the risk of such episodes were identified. A higher incidence of AH and a number of other risk factors, an unfavorable prognostic value of AH in patients after ischemic stroke compared with patients after MI, and a low level of achievement of target values of BP with similar medical and socioeconomic determinants in both groups was demonstrated. Based on the obtained results, practical recommendations for improving the situation with AH control in patients after MI have been developed.
