Lavrinenko A. Optimization of diagnostics and surgical treatment of hematomas mesentery with closed abdominal trauma.

The dissertation is devoted to solving the problem of improving the results of treatment of patients with mesenteric hematoma in a closed abdominal trauma by improving diagnostic algorithm and surgical treatment using minimally invasive methods to reduce postoperative complications and mortality. The work is based on the analysis of results of surgical treatment of 76 patients with CAT. Implementation of tactical algorithms based on the application of differential diagnostic and treatment approach, the use of minimally invasive techniques, allowing a statistically significant lower mortality rate from 37.5% to 13.9%, the average length of stay in hospital patients with 28,17 bed days to 20.86 and to reduce postoperative hospital stay from 24.28 to 16.29. Developed diagnostic algorithm. Resulted in the increase in information content to 38.6 ± 2.2% in conventional methods of diagnosis to 94.3 ± 4.5% when using modern technology, which led to a decrease in the frequency of diagnostic errors from 29.6% to 8,7%, diagnostic decrease search time from 1.55 hours to 0.9 + 0.67 + 0.05, and a reduction resuscitative diagnostic step 2.7 times. The results obtained in the course of experimental and clinical research data allowed justify complex of intensive therapy reduces the amount of non-infectious complications from 24.2% to 20.0% and purulent - inflammatory with 45.5% and 20.0%.
