Object - violation of the small intestine in terms of acute blood loss complicated by ischemia-reperfusion of the limb; goal - to determine the role of ischemia-reperfusion of the limb in the development of metabolic, functional and structural disorders of the small intestine in conditions of acute blood loss and to evaluate the effectiveness of carbacetam in the correction of identified disorders; methods - experimental; biochemical; morphological; mathematical and statistical; results - the role of limb ischemia-reperfusion in the pathogenesis of enteral dysfunction after acute blood loss after 1, 2 hours, 1, 7 and 14 days of the reperfusion period, the effectiveness of carbacetam in reducing the manifestations of the identified disorders; it is shown that due to two-hour limb ischemia in the reperfusion period compared to control in the small intestine after 1 day significantly slows down the absorption function of the small intestine by D-xylose test, which after 7 days normalizes and reaches the level of the control group; It was found that modeling of ischemia-reperfusion of the limb, acute blood loss and their combination causes translocation of intestinal bacteria into the abdominal cavity with a maximum after 1 day of the experiment; It was found that due to two-hour ischemia and reperfusion of the limb in the wall of the small intestine increases the activity of lipid peroxidation, which increases the content of diene and triene conjugates and reagents to thiobarbituric acid with a maximum after 1 day reaches the reperfusion period. ; it is shown that in the conditions of modeling only ischemia-reperfusion of the limb in the wall of the small intestine compared to the control increases the activity of the enzymatic link of antioxidant protection, while after acute blood loss and especially its combination with ischemia-reperfusion of the limb antioxidant protection decreases; It is proved that the use of carbacetam for 7-14 days in the reperfusion period in animals with acute blood loss complicated by ischemia-reperfusion of the limb, compared with animals without correction, causes a significant decrease in the activity of lipid peroxidation and increase the value of antioxidant-oxidative reduction. fractions of molecules of average mass (238-280 nm) in blood serum, improvement of absorption function of a small bowel, essential decrease in species and quantitative structure of intestinal microflora in exudate of an abdominal cavity.
