Shotadze D. Regional blood flow as the factor of an incompetence of intestinal anastomoses: disorders and correction

The clinical and experimental research was conducted with the purpose of development of prophylaxis methods of intestinal sutures and anastomoses incompetence. In experiment on 30 white rats with the help of submicroscopies are detected ultra structural changes of anastomosis zone in early postoperative period, including after using of vasoactive drugs. In clinic with the help of reography, laboratory and clinical researches the role of microcirculation disorders in a pathogeny of intestinal suteres and anastomoses incompetence is confirmed. Was inspected 281 patients, which surgeries with formation of one and more anastomoses divided into two groups - in control group (217 patients) the traditional therapy was conducted, in main (64 patients) - were applied methods of purposeful correction regional blood flow. The offered preventive measures (durable peridural blockade by a solution of Lidocainum and durable novocainic blockade of anastomoses mesentery) had allowed to reduce frequency of anastomosesi ncompetence and postoperative mortality. The methods were introduced into the practice of surgical departments of KRCOD and Kupyansk CRH.
