.Object of the study: genital endometriosis. Purpose of the study: complex investigation of pathogenesis mechanisms of genital endometriosis with the consideration of immunological, hormonal, morphofunctional, metabolic changes in endometrioid heterotopies and in the woman's organism and development of rational therapeutic tactics aimed at reduction of the frequency of illness and increase of the efficacy of treatment of the patients with genital endometriosis on the basis of the obtained findings. Methods of the study: to achieve the purpose of the work clinical laboratory, clinical instrumental, hormonal, immunological, biochemical, histological, histochemical, morphometric, immunofluorescent, immunohistochemical, immunoenzyme, radioimmunological and statistical methods were used. Practical significance of the obtained results: The obtained data about the pathogenesis of genital endometriosis allow to extend the existing criteria for the diagnosis of the degree of endometriosis severity and therapeutic capabilities as well as to outline new approaches to differentiated therapeutic measures in patients with endometriosis. Basing on the findings of the research, a program of differentiated treatment of the patients with genital endometriosis depending on localization and degree of the process severity which includes drug and tissue therapy was worked out. This allowed to increase the treatment efficacy, to reduce the incidence of relapses and the duration of the treatment. Novelty of the obtained results: This work solves an important medical social problem of improvement the efficacy of genital endometriosis treatment basing on the differentiated therapeutic approach depending on localization and the degree of endometriosis dissemination. For the first time, an etiopathogenetic concept of endometriosis development was worked out. It unites morphological, hormonal and metabolic changes in the endometrioid heterotopies and the character of local and general immune reactions in the organism of the woman depending on localization and dissemination of the process. The role of immune reactions in endometriosis development as well as their influence on the tissue and cellular immunity in the zone of endometrioid heterotopies were established. The data about cytokine regulation of proliferation processes in the endometrioid elements, the role of autoimmune reactions in endometriosis development, their influence on the character and severity of the clinical course of the disease were obtained. The character of metabolic processes in the foci of endometrioid heterotopies was determined. For the first time, heterotopic transplantation of fetal thymus was used to correct the immune disturbances (Patent No. 31736A), differentiated therapeutic measures on correction of immunological and hormonal changes depending on the form and degree of the process generalization were worked out. Introduction: The findings of the research are used in the work of gynecology departments of Kharkiv Specialized Maternity Center No. 5, Kharkiv Regional Clinical Hospital, Kharkiv Center for Family Planning and Human Reproduction, maternity hospitals No. 2, 3, 6, 7 of Kharkiv, Kharkiv region, Ternopil. Theoretical concepts and practical recommendations of the work are included in the lectures and practical classes in gynecology for students and interns of Kgarkiv State Medical University, Kharkiv Medical Academy for Advanced training, Ukrainian Medical Dental Academy (Poltava).
