Essay

The dissertation is devoted to clinical and pathogenetic features of multiple sclerosis (MS) and to elaboration of new approaches concerning diagnosis, prognosis and treatment. Thesis summarizes and offers new solution of the scientific and medical problem, based upon specification of the mechanisms, underlying destructive changes in MS, taking into account clinical features of the ailment. Among patients with MS we studied changes of strucrural and functional state of osseus tissue, activity of specific izoenzymes and izoforms of alkaline phosphatase, tumor necrosis factor-a and interleukine-6, protein-glycoprotein fractions of blood serum and erytrocyte membranes. We described cytopathomorfological alterations in lymphocytes of peripheral blood among MS patients. It was determined important role of apoptosis in MS pathogenesis. We figured out DNA-fragmentation of mononuclear cells of peripheral blood and described diminution of nitrites content in T-cells medium of peripheral blood. Also it was proven clinical efficiency of pulse-therapy with glucocrticoids in MS, because of which occurs local elimination of autoreactive T-cells through induction of apoptosis. We proposed new complex approach to diagnosis and differentiated treatment in MS.
