Object of the research: Portal hypertension m cirrhosis of the liver Purpose of the research: To improve the results of treatment in patients with portal hypertension complicated with ascitic syndrome by means of developing and introduction to clinical practice of the methods of complex staged treatment of this category of patients Methods of the research General clinical, clinical laboratory, biochemical, immunological Instrumental methods (electrocardiography, electroencephalography, cardiosynchromzed Doppler study of porto-hepatic circulation, ultrasound study, radioisotope scanning, angiogra-phy, fibrogastroscopy, retrograde cystometry, spirography), statistical methods Practical significance of the obtained findings: A minimal invasive endovascular method of embolization of the left gastric vein together with the left gastnc artery was introduced to the clinical practice Optimum surgical approach to the thoracic lymphatic duct on the neck was introduced This promotes lymphovenous anastomosis and ductolysis A method of dialysis-sorption of ascites fluid with remfu-sion of preliminary frozen ultrafiltrate and a method of albumin peritoneal dialysis were introduced Clinical capabilities of endovascular correction of disturbances of hemo- and lymphodynamics in cirrhosis of the liver complicated with ascites were investigated Novelty: The scientific concept of poly-syndrome development m patients with cirrhosis of the liver complicated by ascites was expanded For the first time, it was established that elevation of the mtraabdominal pressure results m worse blood supply to the liver due to reduction of volume perfusion of not only portohepatic vessels but also inferior vena cava and renal veins For the first time, complex staged treatment of the patients with cirrhosis of the liver complicated by ascites was suggested, which allowed to correct the dominating syndrome at each stage of the disease An original technique of albumin peritoneal dialysis was suggested The advantages of remfusion of preliminary frozen ultrafiltrate of ascetic fluid after its dialysis-sorption were proven A new technique of approach to the thoracic lymph duct from the cervical approach was worked out in the experiment Anatomical variants of the left gastnc vein location were established Degree of introduction: The findings of the research are used in surgery department of Regional Clinical Hospital, Central City Hospital (Uzhgorod), m the teaching process at medical faculty of Uzhgorod National University Sphere of application: Medicine, surgery
