Essay

The investigation object: pathogenesis of chronic inflammation. The investigation goal: finding out of the cellular and humoral hematological mechanisms of inflammation chronization, substantiation of hemomodulator application for prophylaxis and treatment of chronic inflammation. Methods of research: histological, histochemical, immunohistochemical, hematological, immunoenzymatic, clinical-laboratory, functional, statistical. The theoretical and practical importance of the received results: the obtained results expand existing concepts of mechanisms of inflammation chronization and can be used in further research and teaching. It is shown also, that CSF contents in blood can be used for early diagnosis of chronization of acute inflammation, and also for differentiation of primarily and secondary chronic, immune and non immune chronic inflammation (CI). The method of diagnosis of inflammation process chronization based on the ratio between G-CSF and GM-CSF in peripheral blood was developed, on which apatent of Ukraine was received. The efficacy of application of recombinant human G-CSF – neutropogenum was estimated in complex anti-inflammatory therapy of chronic inflammatory diseases. It opens new prospects in diagnosis, prophylaxis and therapy of CI, and also demonstrates advisability of the further clinical-experimental research at choosing of hemomodulators, methods, schemes and doses of their application. The scientific novelty of the investigation results: there were estimated new hematological mechanisms of inflammation chronization; it is shown, that first it is characterized by the greater duration of neutrophil infiltration and intensifying of Т-lymphocyte infiltration; the latter precedes macrophage and fibroblastic reactions. The expression of CD54 and especially CD44-antigens is decreased in the inflammatory focus, expression of CD11c-, CD31-and CD61-antigens becomes more intensive. The following changes occur: repeated activation of hemopoiesis, output of leucocytes into blood, phase changesof CD-positivity in bone marrow, increase of CD11c+-, CD3+-, CD4+-, CD25+-, CD45RA+-cells, less expressed decrease of CD71+-, CD61+-cells. Initial decrease of neutrophil and lymphocyte activity and increase of monocyte activity in blood are observed, latter on – a decrease of monocyte activity, increase of lymphocyte activity, and increase of need and lack of CSF are observed. On that basis a possibility of hemomodulator application for prophylaxis and treatment of CI was experimentally shown for the first time; the efficacy of G-CSF application is demonstrated and mechanisms of its anti-inflammatory action are discovered in CI, just caused by an increased production of functionally active leucocytes. The degree of implementation: the results of the research are implemented in the educational process at departments of pathophysiology of Kharkov, Bukovin, Donetsk, Lugansk, Odessa, and Ternopol State Medical Universities.
