Dovbysh M. Pathogenetic meaning of lymphatic system failure in upper urinary tract obstruction and kidney inflamatory process and their treatment.

Dissertation is devoted to the study of questions of patogeneticheskogo value of violations of the lymphatic system in development of sharp obstructions of overhead urinary ways and sharp pyelonephritises and development of methods of limfostimuliruyushey therapy and methods of surgical medical treatment. The results of medical treatment of 232 patients with the sharp obstructions of overhead urinary ways and 221 patient on sharp pyelonephritises and experimental data conducted on 75 rabbits are represented. It is shown, that at the sharp obstructions of overhead urinary ways and sharp pyelonephritises there is violation of limfodinamiki general and regionarnoy: speed of rezorbtsii diminishes and is increased time of eliminatsii of limfotropnih dyes at intradermalnom and regionarnom introduction under the fibroznuyu capsule of bud. In the early (2-14 days) terms of violation of outflow of lymph from a bud in the experiment on rabbits there is the edema of fibroznoy capsule, parenhimi of bud and paranefralnoy cellulose, violations of gemomikrotsirkulyatsii and expansion of lymphatic vessels, dystrophy of epithelium of kanaltsev and satiation by an interstitsiya albumen. In 30- 60 days there is a plenty of connecting fabric round vessels, balls and between kanaltsami. At intravenous introduction to the rabbits E. Coli, on a background broken outflow of lymph, there are limfotsito-leykotsitarnie infiltrati of paravazalno and between kanaltsami, is special on a crust-cerebral border. In 80% of cases arises up nekropapillit, and at simultaneous ligirovanii ureter a festering-destructive process develops in a bud - pionefroz. The method of surgical medical treatment of patients with a sharp festering pyelonephritis by kapsulotomii and drenirovaniya of subcapsule crack is developed, that allows to save anatomo-fiziologicheskuyu integrity of bud and lymphatic river-bed of fibroznoy capsule and to decrease the amount of nefrektomii to 5,9%. The regionarniy method of limfostimulyatsii at patients on a sharp serozniypyelonephritis after operative interference and method of limfostimuliruyushey therapy is offered at patients with the sharp obstruction of overhead urinary ways and testimonies and contra-indications to their use are developed.
