The thesis is devoted to the issues of increase of treatments’ efficacy of prostate cancer patients by development of conception differential diagnosis, namely development with the help of mathematical model of objective prognostic criteria of treatments outcomes of prostate cancer and correction’s principles of patients status, improvement of the existing methods of surgical treatment. For establishment of prostate cancer preliminary diagnosis or prediction of treatments outcomes in a patient the value of diagnostic coefficients (DK) of his indexes of organism and parameters of vesico-urethralis segment, which summarize in accordance to diagnostic and prognostic tables, is calculated. Under reaching a diagnostic threshold which is evened «+» 130, with the 95% theoretical accuracy the preliminary diagnosis of benign prostatic hyperplasia is set, or favorable outcome of early and remote postoperative period (the group of low risk of complications and 5-years lethality); at the DK sum is «-» 130, preliminary diagnosis in a patient is prostate cancer and there are indications for its biopsy, and a patient with prostate caner attributes to the group of high risk with the unfavorable outcome of radical prostatectomy; if the DK sum achieves neither of thresholds, a diagnosis and prognosis consider indefinite (group of middle risk). For the receive of favorable outcomes of radical prostatectomy, determination of optimum parameters of initial somatic status of patient with prostate cancer before and after the operation – must be based on quantitative indexes – the maximal positive values of DK. The designed methods of diagnosis and prognosis of treatments results of prostate cancer, their developed computer versions and improved method of retropubic radical prostatectomy allow to optimize and individualize medical tactic in patients with prostate cancer, to multiply the quantity of favorable early and remote results on 29% and 45%, accordingly.
