It is conduct complex research of methods of diagnostics and treatment 211 patients with extrasphincteric rectal fistulas (ERF). It is set that this pathology often meets in the most capable of working age. Developed an author and inculcated in practice fistulography with the use of by an anorectal straightedge which allows to multiply the indexes of diagnostic value of method First developed by author and inculcated in practice technology of transrectal ultrasonic research with the three-dimensional-spatial reconstruction of image in the diagnosticians of ERF, the high diagnostic value of this method is proved. On the basis of results of transrectal ultrasonic research of patients with ERF working US-classification is developed them after the degree of complication It had been lead the statistical analysis of diagnostic value of methods of preoperative topographer-anatomical diagnostics of patients with ERF, on the basis of which was developed and grounded diagnostic algorithm for these patients. Itwas offered two new techniques of treatment of this category of patients: method of the "integrated block" and a technique of "packing" with autothrombin glue. It is proved, that the offered techniques of treatment give authentically (p<0,05) best result, compared with traditional techniques. It has grounded a medical algorithm at ERF.
