An object The burn disease and its complications in of till seven years children; the purpose The improvement of extensive superficial and deep burns treatment results in children of younger age by the system engineering of preventive surgical treatment, directed by preventive MODS maintenance and scars consequences by studying on basis of the stress-reactions and adaptable mechanisms in the burn disease sharp period; methods - common - clinical, laboratory, tool, mathematical, statistical; the novelty - Substantiation of a new direction in a problem of extensive burns treatment in children of younger age by development of the diagnostic and tactical approach to superficial dermal burns immediate debridement and closing with the biological coverings already in a burn shock stage and deep burns early esharectomy and wounds closing, on the depending area, due to autodermal plastic combined with the cellular technologies usage.Active surgical tactics is effective in maintenance of the MODS prevention dueto: reduction of an ischemia in tissues, the effective mechanisms of indemnification hemodynamical system and microcirculation activation, preservations of viability of a great volume of integumentary tissues, increases of an organism reactance and septic complications probability reduction.The developed surgical tactics productivity within the limits of the concept « total continuous burn wounds closing» is proved;the results: The number of sepsis cases has decreased by 1,7 times. The average hospital stay has decreased by 1,8 times, by 1,5 times at more severe burns. Due to tactics of surgical treatment the scar deformations quantity were managed to decreased from 42,1 % to 18,2 %; it is inculcated - in medical practices, in an educational process; the sphere is a medicine.
