Posternak G. Finding out of features of pathogeny of burn shock and optimization of his intensive therapy for children (experimental-clinical research )

The dissertation is devoted optimisation of an intensive care of a burn shock at children by finding-out of the basic pathogenetic mechanisms of formation of the system inflammatory answer at a thermal trauma. It is established, that the thermal trauma of serious and extremely serious degree is accompanied by the multifunctional disturbances, expressed metabolic discoordination and hypoxia at all victims, especially at children of early age. The serious burn trauma caused the strengthened local and system synthesis of proinflammatory cytokines in children throughout all period of a burn shock with their maximum level in first two days after a trauma, especially at damage of children by boiled water and a water steam. It is proved, that the syndrome of the system inflammatory answer is most expressed at duration of the shock period till 72 o'clock. It is pathogenetically proved complex program of treatments which includes active respiratory tactics and organprotection's therapy which allows regulate a current of the system inflammatory answer, to warn development of multiorgan insufficiency and to improve results of treatment of suffered children with a thermal trauma
