The dissertation is devoted an actual problem of a choice of a method of the miniinvasive surgical treatment complicated peptic ulcers of the pyloroduodenal zones. The profound research of the reasons of negative results surgical treatments peptic ulcers has allowed to develop algorithm of a choice of volume miniinvasive surgical intervention taking into account age of the patient, balance of vegetative regulation, level changes gastrointestinals hormones and a condition motorno-evakuatornoj stomach functions. Application the algorithm of a choice proved performance at 73.7% vegetative denervashions in the combination with drainages pyloroduodenal zones. Stenosis plastics on a frame drainage were thus applied a new way sympathetic denervations in volume laparoscopics selective symphatectomiy and highly effective a method. The substantiation of a choice of a method and application miniinvasive surgical intervention at patients with complicated peptic ulcer of the pyloroduodenal zones have allowed ways dicrise in 4.3 times quantity postoperative complications and to lower in the remote period after operation on 4% frequency of relapses of an ulcer and in 2 times pathological changes in pyloroduodenal zones.
