The paper provides theoretical and experimental synthesis of the important scientific problem. It is developing advanced immunological and molecular-biological diagnostic methods for study the age pathology of bone tissue. The importance of the role of the proinflammatory phenotype of the immune system in the development of postmenopausal osteoporosis was substantiated, and the search of immunological and molecular-biological markers, which are responsible for age-related changes of structural and functional state of bone tissue, was performed. As a result, (a) the key indicators of the immune system that change in patients with osteoporosis were found, (b) the hypothesis about the role of proinflammatory phenotype of the immune system in the mechanisms of development of age pathology of bone was experimentally confirmed, (c) immunological and genetic risk factors for osteoporosis and fractures in postmenopausal women were established.
