Essay

The dissertation focuses on improving outcomes of children with extensive burns. Program of surgical treatment designed and pathogenetically reasonable. The results had of examination and treatment of 415 patients. Correlation was found between the course of acute period of burn disease, performance of clinical and laboratory parameters, inflammatory markers and anti-inflammatory response, demonstrated the positive impact programs of surgical treatment of common burns in the course of systemic inflammatory response. High efficiency programs of surgical treatment on the course of intoxication syndrome and oxidative distress, the positive influence on the development of immune responses and nonspecific resistance. Investigated parameters allowed to predict possible development of infectious complications. At first time defined the terms restoration of skin, which prevents the development of septic complications. The programs of surgical treatment have improved the treatment results: total bed-days of patients with medium severity fell on 5 days, severe patients on 10.6 days, extremely severe patients on 11.1 days, term recovery of the skin - on 2.5, 9.5 and 7.4 days, incidence of infectious complications in 2.9, 1.5 and 1.6 times, frequency of early contractures and scar deformities decreased in 2.5, 1.8 and 1.6 times, mortality of extremely severe patients decreased from 1.69% to 0.42%.
