Zozulia A. Medical Social Substantiation of Qualitatively New System of Specialized Care for Patients with Cerebrovascular Pathology.

The dissertation is devoted to actual scientific problem: the substantiation of qualitatively new complex healthcare system for city adult population in case of cerebrovascular diseases. Complex medical and social study found inconformity of the existing system of medical care in case of cerebrovascular diseases with the city population needs, medical care reformation, the best international experience and proven efficacy data. There was developed qualitatively new complex healthcare system in case of cerebrovascular diseases. Based on primary and secondary prophylaxis and cross-sectoral approach, the system determines preventive stages and coherence, provides up-to-date organizational and clinical technologies implementation by introducing loc clinical protocols for a particular area. The central element of the system is its restructuring process with the involvement of primary medical and sanitary care, creating neurovascular centres, specialized rehabilitation departments, cross-sectoral medical teams to provide medical care for patients at home. The medical team functions must be based on the use of proven efficacy data and outcome evaluation indicators which, when integrating with previously existing and functionally improved elements, provided complex staged health care in case of cerebrovascular diseases and their complications. The suggested system was proven to be effective according to the results of implementing its separate elements.
