Essay

Object of research - genital, extragenital herpes, herpes-associated erythema multiforme exudative. The purpose of research - to increase effectiveness of diagnosis, treatment, prevention of dermatoses associated with herpes simplex viruses (exudative erythema multiforme, eczema, Kaposi eczema), based on new data on their epidemiology, clinical presentation, immunopathogenesis and morphogenesis. Methods: clinical; epidemiological analysis methods; immunofluorescence and immunoenzyme detection of antigens and antibodies; molecular genetics; biochemical; immunological; histological, histochemical and cytological methods; electron microscopic examination; biometric and medical statistics. The Dissertation is devoted to dermatoses associated with herpes simplex virus (exudative erythema multiforme, herpes eczema Kaposi). The high incidence of herpesvirus DNA (81,7±4,3%) shows their etiologic role. Specific IgG antibodies against Herpes simplex virus I and/or II were found in 84,1±4,3% of patients. The virus directly damages tissue and triggers autoimmune response. Electron microscopic examination revealed necrosis of epithelial cells, loss of cell-to-cell contacts, intracellular morphological structures of viruses are observed. Significant imbalances of various cytokines concentrations were revealed (tumor necrosis factor-alfa, interleukin-1alfa, -4, -6, -8, interferon-gamma). The effective treatment was proposed (valacyclovir - 500 mg 2 times a day, 15 days, immunoglobulin against herpesvirus, two courses 5 injections each, membrane plasmapheresis - 4-7 procedures). A prevention program of Herpes simplex virus prophylactics was suggested. Results have been implemented in the practice of ME "Zaporizhzhia Regional dermatovenerologic clinical dispensary", ME "Odessa Regional dermatovenerologic dispensary", Poltava Regional Clinical Dermatovenerologic Dispensary, ME "Rivne Regional dermatovenerologic dispensary", MI "Regional cutaneous Venereal Diseases" (Dnipropetrovsk), MIH "Regional Clinical dermatovenerologic dispensary no. 1" (Kharkiv), MCTPEH "Regional dermatovenerologic dispensary" (Donetsk). Research results are used in the educational process of postgraduate education of dermatovenerologists: at the Department of Dermatology and Venereology and HIV/AIDS of Kharkiv Medical Academy of Postgraduate Education, at the Department of Dermatology and Venereology of Odessa National Medical University, department of Dermatology and Venereology of National Medical University named after O.O. Bogomolets, the Department of Dermatovenereology and Cosmetology with Cycle of Aesthetic Medicine of Faculty of Postgraduate Education of Zaporizhzhia State Medical University.
