Essay

The thesis is dedicated to improving the results of surgical treatment of patients with generalized peritonitis (GP), by the application of the proposed program of monitoring diagnostics for early prediction and early diagnosis of postoperative intra-abdominal complications, optimizing the surgical approach, the use of targeted immune and developing a method for the prevention of anastomotic leak in the postoperative period. The analysis of the results of treatment 355 patients with GP, taking into account the severity of the patients on the scales SAPS, APACHE-II, MPI and SIRS, with the study of immunological parameters, cytokine and antioxidant status of endogenous intoxication and acute phase proteins in various biological media. Application of the proposed three-tier cytokino- and ozone therapy, intra-abdominal regional endolymphatic therapy, forecasting algorithms postoperative inflammatory complications and choices designed to reduce the frequency of immune allowed relaparotomies with 11,1±2,3% to 4,6±1,6%, postoperative mortality to 25,0±3,2% to 17,7±2,9% and postoperative infectious complications with 47,8±3,7% to 22,3±3,1%.
