The consistent patterns and typology of pathomorphosis of symptomatic epilepsy in comparison with cryptogenic and idiopathic ones were established by exploring factors that determine its development. The following types of epilepsy pathomorphosis were identified: etiological, clinical (with subtypes - neurological, psychiatric, somatic), age-related, therapeutic and socio-demographic ones. Typology of pathomorphosis is considered from the standpoint of a positive or negative impact on the patients' state of health depending on various factors that modify it. It was stated that cryptogenic epilepsy in its course has many common features with symptomatic epilepsy, allowing to identify common types of pathomorphosis for both etiological forms of the disease. The effect of comorbid pathology (headaches, acute and chronic somatic diseases, psychiatric disorders) upon clinical and therapeutic pathomorphosis of epilepsy was studied, whereby systematization of comorbidity in epilepsy has been proposed.The reasons of remission failures and formation of conditional pharmacoresistance were studied with their division into iatrogenic, astenogenic and independent. To improve the remote prognosis of epilepsy the necessity of early adequate treatment at disease onset was proved.
