Dissertation deals with the issue of the treatment effectiveness in patients with intracranial hemorrhage of aneurism genesis by means of modern approaches used in the perioperative period to prevent and treat secondary brain ischemia as well as the development of its complex correction using statins, prophylactic and curative hypothermia. Theoretical aspects of the scientific issue solution and general results of prospective clinical research in 328 patients with intracranial hemorrhage of aneurism genesis have been presented. The offered methodology of hyperglycemia and hypokalemia prevention in the setting of hypothermia. It has been found out that in 6 months after operation the usage of prophylactic hypothermia leads to the reduction in number of patients with severe invalidisation, therefore, can be recommended for further use in patients without hypertonic disease. It has been proved that therapeutic hypothermia improves both immediate results and late fate of treatment in patients with intracranial hypertension and secondary brain ischemia due to the reduction of lethality, improvement of neurological condition and cognitive functions at all stages of observation as well as 5-year function of surviving. It has been stated that three main groups of postoperative factors influence the prognosis; algorithms of complications prognosis and treatment results have been developed.
