Ishchuk V. Clinico-psychological characteristics of psychendocrine syndrome in pathology of thyroid gland, adrenal cortex and gonads (semiotics, typology, psychoprophylactic support)

The dissertation thesis is solving an important scientific problem in field of medical psychology, endocrinology, psychiatry and psychotherapy: on the basis of the analysis of psychosocial and clinical psychopathological features of the psychoendocrine syndrome in patients with hormonal exchange disorders of the thyroid gland, adrenal cortex and hypogonadism, established its structural features and clinical and psychological typology; formed a type-specific contrainozogenic and contrasemiotic system of psychotherapy which considering the clinical-psycho-phenomenological features of patients. A study was conducted on the contingent of 400 patients undergoing treatment on the basis of the Ukrainian Scientific and Practical Center of Endocrine Surgery, transplantation of endocrine organs and tissues of the Ministry of Health of Ukraine for the hormonal exchange disorders of the thyroid gland, adrenal cortex and hypogonadism with diagnosed psychodenocrine syndrome. The study was implemented using clinical-anamnestic, psychodiagnostic, clinical-psychopathological, clinical-psychophenomenological and clinical-statistical methods in 4 stages. The levels of representation of psychogenic triggers of the manifestation of endocrine pathology in patients with thyroid gland, adrenal cortex and hypogonadism have been revealed. The spectrum of nosogenic factors of psychotraumatism was revealed, dynamic organization, nosological structure and representation of nosogenic factors of psychotraumatism in contingents of patients at different stages of endocrine pathology were established. Detected nosogenic factors of psychotraumatisation are systematized in 3 groups: valiocentric, sociocentric and anhedonic. The medical-psychological and clinical-psychopathological typification of the psychodenocrine syndrome and the comparison of the manifestations of the latter in the groups of patients were performed. Based on the comparison, the psychopathological and psychosomatic semiotics included in the structure of the psychendocrine syndrome were distributed to the hormone-dependent (permanent) and hormone-independent (modifying). On the basis of clinico-psychophenomenological examination of contingents using the methodology of evaluation of the psychophysical profile, nosological-associated with endocrine pathology types of the latter with the subsequent selection of the corresponding psycho-technical support were established. Based on the received data, a program of psychoprophylactic support for patients with thyroid gland, adrenal cortex and hypogonadism and hypogonadism from psychendocrine syndrome was established.
