Essay

The dissertation is devoted to the problem of prevention of the syndrome of multiorgan failure in polytrauma by developing and implementing in practice the methods of volemic support and nutritional support. The analysis of the dynamics of clinical, biochemical and instrumental parameters in 170 patients with polytrauma was performed. A number of indicators have been identified that significantly correlate with the development of sepsis: hyperthermia, systolic blood pressure level, infusion volume, hyperbilirubinemia, acidosis, PaO2 level. The method of determining the effectiveness of infusion therapy in patients in the acute period of polytrauma, the method of early determination of severity and prediction of polytrauma, the method of nutritional therapy of patients with polytrauma by early enteral nutrition with expanded protein-calorie supply. The use of implemented intensive care regimens allowed to achieve early normalization of central hemodynamics, hourly urination and diuresis at a significantly lower 8.9% daily infusion volume, reduce the risk of MODS, reduce the duration of treatment in ICU by 15.7%.
