In order to predict, diagnose and prevent perinatal pathology in pregnant women after an application of assisted reproductive technologies, a retrospective analysis of 221 histories of pregnancy and childbirth of pregnant women and women in labor after ART was carried out in the work according to archival material. At the second stage, 299 pregnant women were prospectively examined, 249 women of the main group - pregnant women after ART and 50 women with spontaneous pregnancy - the control group. Depending on the factor of infertility, we distinguished - Group I - 94 women with tubo-peritoneal factor of infertility, Group II - 87 women with endocrine factor of infertility, Group III - 68 women whose infertility is caused by the male factor. Hormonal, psycho-emotional state, feto-placental complex, biocenosis of genital tracts, level of homocysteine, vascular-endothelial growth factor, endothelin-1, immunological and coagulation links of homeostasis were determined for all pregnant women. At the third stage - the women of the main group were divided into subgroups: subgroup (I-III) A - pregnant women who received the proposed treatment and prevention complex and complex of psycho-emotional correction, subgroup (I-III) B - pregnant women who were observed in accordance with generally accepted standards. An analysis of the course of pregnancy and childbirth and the condition of the fetus and newborn in accordance with the selected management tactics was also carried out. As a result of the work, it was established that the frequency of infertility increased from 20 (9.0%) in 2013 to 66 (29.9%) in 2017. In the structure of the examined, pregnant women over 35 years of age with the prevalence of combined somatic and gynecological pathologies, and the frequency of perinatal mortality was 18.0‰. In the anamnesis of pregnant women after ART, the presence of infectious, somatic and gynecological diseases was established. The course of pregnancy in women after ART in the I trimester was complicated by the threat of miscarriage, in the II and III trimesters by the threat of late spontaneous miscarriage, premature birth, isthmic-cervical insufficiency, preeclampsia, placental dysfunction, and vaginitis. The main links of homeostasis in pregnant women after the ART: insufficient β-HCG production, a decrease in the levels of progesterone, an increase in the level of estradiol, cortisol and prolactin; Psychoemotional state - an increase in cases of high levels of reactive and personal anxiety, depressive, anxious and euphoric types of gestational dominance; In utero-placental hemodynamics: an increase in the systolic-diastolic ratio in the 1st trimester, arterial and venous types of blood flow, increased blood flow in the umbilical artery; With an increase in the level of homocysteine; changes in coagulogram indicators. The positive effect of the proposed complex is realized due to the normalization of: hormonal homeostasis - an increase in the levels of β-HCG, progesterone; decrease in estradiol, cortisol and prolactin levels; psycho-emotional state: absence of cases of high reactive and personal anxiety in women of subgroups IA, IIA, IIIA, absence of cases of depressive and anxious types of gestational dominance (р<0.05); uterine-placental hemodynamics: normalization of blood flow in the uterine arteries and reduction in the frequency of cases of placental dysfunction and abnormal amount of amniotic fluid; biocenosis of the genital tract: reliable registration of a sufficient number of Lactobacillus spp (р<0.05); normalization of homocysteine concentration in pregnant women subgroups IA, IIA and IIIA (р<0.05); coagulation homeostasis: normalization of indicators of fibrinogen, D-dimer and soluble fibrin-monomer complexes (р<0.05 ); endothelial status: a significant decrease in the value of VEGF and endothelin-1 (р˂0.05). The criteria for early diagnosis of disorders of maternal-fetal hemodynamics should include: an increase in the systolic-diastolic ratio in the uterine arteries and the appearance of arterial-like blood flow in the intervellous space. The criteria for a negative prognosis of the course of pregnancy include the discrepancy of β-HCG with the term of pregnancy, progesterone, the estrogen/progesterone ratio and an increase in the concentration of cortisol and prolactin starting from 3-4 weeks of pregnancy; increase in concentration of homocysteine, D-dimer, VEGF, endothelin-1.
