Essay

The dissertation is devoted to the improvement of diagnostics of combined pathology of chronic pancreatitis (CP) and hypertension (HT) by studying common ethiopathogenetic factors, features of clinical course of diseases, extension of standard diagnostic spectrum and optimization of treatment by supplementing the standard drug regimen. In order to solve the tasks set in the dissertation, a comprehensive general clinical, laboratory and instrumental examination of 102 patients with CP in combination with HT of the II stage, undergoing inpatient treatment at the Khust district hospital in 2017-2018, was performed. The basis for the division is the principle of treatment correction. Group I (n=53) received basic therapy (BT) in accordance with the requirements of the clinical protocol. Treatment of the group II (n=49) was limited to BT with optimization (OT) by including the following drugs: “Zinc selenium active”, “Folic acid”, and “Epadol-neo”. To compare the results, a control group (almost healthy individuals) was formed, representative by sex, age and place of residence (n = 23). Confirmation of the feasibility and effectiveness of the OT revealed a difference in the dynamics of the main clinical symptoms and the results of laboratory and instrumental research. Statistically significant difference between groups was achieved in the following clinical symptoms: abdominalgia, nausea, decreased appetite, heaviness in epigastric region, flatulence, stool changes, signs of hypovitaminosis, and abdominal tenderness during deep methodical palpation (р<0,05). Due to the prescription of OT was achieved reliable dynamics of a number of indicators of lipid metabolism (reduction of levels of TG, LDL CH, VLDL CH, IA, Apo B/Apo A1 and the increase in anti-atherogenic components of lipid metabolism: HDL CH, Apo A1(p<0,05)). On the contrary, in cases of patients in group I, significant changes were noted in the levels of HDL CH, IA and Apo A1(p<0,05). Increase in the concentration of Ca, Se and Zn (p<0,05) was certain in the cases of patients of the group II in the contrast with group I. Significant intergroup difference in the final levels of the parameters was indicative: C-reactive protein, fibrinogen, IL-6, IL-4, glutathione peroxidase, Se, Zn, transferrin, folic acid (p<0,05).
