Essay

The present dissertation thesis deals with solution of the critical scientific and practical problem of modern prosthetic dentistry, being the improvement of the algorithm of diagnostics and personalized approach to prosthetic treatment of 9 patients with the help of fixed prosthetic restoration by way of clinical and instrumental establishment of the effectiveness of registration materials (RM) for bite registration, and application of methods for establishing intermaxillary relations in the position of maximum intercuspation (MIC) (cusptofissure occlusal contacts), IRMI. The comparative analysis of clinical and instrumental indicators of patients of the treatment group conducted before and after dental prosthetic rehabilitation with fixed dentures highlighted that with the use of Futar D RM, the median values of the parameter of occlusal loading asymmetry of the right and left sides of the dental arches decreased by 2,3 times (up to 10,00 %) at IRMI, by 1,17 times (19,00 %) at ICP, and by 1,38 times (up to 28,60 %) at ∆. With the use of Consiflex material, the median values of the parameter of occlusal loading asymmetry of the right and left sides of the arches decreased by 2,26 times (up to 10,00 %) at IRMI, by 1,38 times (up to 17,00 %) at ICP, and by 1,32 times (up to 17,00 %) at ∆. While using metalized wax, the median values of the parameter of occlusal loading asymmetry of the right and left sides of the arches conversely increased by 1,63 times (up to 15,40 %) at IRMI, by 1,08 times (10,20 %) at ICP, and by 5,11 times (up to 26,60 %) at ∆. The comparative clinical and digital instrumental studies of the methods and materials of MIC recording applied allowed establishing in an objective way that the use of Futar D and Consiflex RM in the said sequence effectively ensures the stability of spatiotemporal parameters of digital occlusion characteristics in IRMI for fixed restoration in patients with bounded and end edentulous spaces. Dental restoration in patients of the first and second treatment groups was 14 carried out by traditional prosthetic rehabilitation with porcelain crowns and porcelain fused metal and porcelain bridge restorations on prepared natural teeth (9 patients) and dental implants (11 patients). Prosthetic modification of occlusal tooth surfaces by means of fixed microrestoration was performed in 4 patients with intact teeth after the use of RM in order to establish the IRMI position. Dental prostheses were not used in 16 patients of the control group. Consistent symmetry of occlusal loading of the right and left sides of dental arches in the modes of digital analysis of occlusion at IRMI, ICP and ∆ is best maintained after the completion of prosthetic rehabilitation of patients with fixed dentures fabricated with the use of Futar D RM. The comparative scientific research conducted allowed substantiation of differentiated approach to the choice of IRMI recording methods and the use of RM, and enabled comprehensive formulation of practical recommendations for their use in the presentday dental practice, which significantly improves the quality and promotes the effectiveness of the eventual clinical result of fixed restoration in patients with edentulous spaces.
