The thesis deals with the relevant scientific issue of the healthcare, which scientifically justifies the methods which improve the dental service management, as well as functioning of the public dental healthcare institutions. The author introduces a complex of the managerial approaches to quality improvement of the healthcare institutions – state private enterprises, which provides effective function of the healthcare institutions under the modern economic reality.
The study has been held in seven sequential stages, according to the introduced by the author special research schedule. The author used various general scientific and specific methods, defined in the passport of the specialty “Social medicine” (systemic analysis, bibliosemantic, sociological, medical-statistical, modeling, and expert assessment), collected the information, organized the databases, treated them statistically and analyzed the results, provided medical and social justification and introduced a complex of the organizing method approaches to transition of the public dental healthcare institutions onto the state private enterprise principles, which would improve system of the quality management .
During the first study stage the author reviewed the world and Ukrainian experience on the modern condition and challenges of organizing and management of the quality medical service, both in the world and in Ukraine, and defined basic issues to be further studied.
The first stage, with the bibliosemantic analysis of 276 sources, provided opportunities for generalization of the world experience, detecting problem issues in the scientific justification of methods of the dental service management improvement.
During the second stage, the study direction and objectives were chosen, in order to define the aims, tasks, schedule, methods and scope of the study, as well as its design was produced.
The third stage is represented with analysis of the personnel network, as well as performance of the state, public, and private dental institutions of Kyiv in 2010-2020, according to the statistical reports data (standard form №20, standard № 17, standard 47).
The fourth stage deals with the characterizing organization of the dental service rendered to adult Kyiv citizens, according to the sociological survey data. The survey included 401 patients of public and 389 patients of the private establishments. Within the study, the results of the sociological survey of 259 dentists of public and 109 of private institutions were treated. The author performed an expert assessment of the structural-organizational components of the public and private dental healthcare institutions performance.
During the fifth stage, the concept approaches to transition of the public dental healthcare institutions onto the state private enterprise principles of function, as well as improvement of the quality function management principles, were established.
Within the sixth stage, according to the analysis of the previous stages, a complex of organization methods for advancing the healthcare institutions as state private enterprises’ performance quality management was introduced. This complex is based on two scientifically justified organizational directions: legal organization and manufacture organization; and the block model of management of the healthcare institution as a state private enterprise performance quality, indicators of management quality.
The seventh stage includes introduction of certain components of the suggested complex of the organization methods, aimed at perfection of the management of dental care and quality of the public dental healthcare institutions’ function, assessment of their medical, social and economic effectiveness and expert assessment of their prognostic effectiveness.
