Essay

The object of the research is the process of physical therapy of elderly people with knee endoprosthesis and geriatric syndromes in the remote period after endoprosthesis.The purpose of the study is the theoretical justification, development and verification of the effectiveness of a complex program of physical therapy for the elderly with the consequences of total knee arthroplasty, aimed at improving the functional capabilities of patients and facilitating the performance of activities of daily life by correcting the signs of geriatric syndromes and functional limitations of the operated limb.Research methods. Analysis of special and scientific-methodical literature; to characterize the condition of patients, testing was performed according to the Short Physical Performance Battery and cat dynamometry; physical qualities were assessed by the Senior Fitness Test and the Edmonton Frail Scale; quality of life was characterized by the SarQoL questionnaire; the degree of depression was determined by the Geriatric Depression Scale (GDS-15). Parameters of geriatric syndromes were characterized by the results of Functional Gait Assessment, Fall efficacy scale, Performance-Oriented Mobility Assessment scale; by the Tampa Kinesiophobia Scale; 25-question Geriatric Locomotive Function Scale. The functional state of the knee was determined by the amplitude of movements in the knee joint, manual muscle testing, Forgotten Joint Score-12, Knee Society Score. The obtained materials of each stage of the research were processed by mathematical and statistical research methods.Theoretical and practical significance. consists in the creation of a scientifically based complex program of physical therapy for the elderly with a knee joint endoprosthesis, which contributed to the reduction of the severity of geriatric syndromes associated with muscle weakness, balance disorders, the risk of falling, depression, improving the functional capabilities of the operated limb by reducing the symptoms muscle weakness, improvement of static and dynamic balance, reduction of depression, improvement of quality of life, stability and strength of the operated limb.The practical developments of the dissertation research were used in the practical activities of the Center for Restorative Medicine and Rehabilitation "ARAVMED", the Rehabilitation Center of St. Yuda Thaddeus. Theoretical and methodological developments of the research are used in the educational process of the department of general practice, family medicine and rehabilitation of the Ivano-Frankivsk National Medical University. The program should be used in the activities of physical therapists, occupational therapists, doctors of functional and rehabilitation medicine and other specialists of the multidisciplinary rehabilitation team of specialized rehabilitation departments and centers of trauma-orthopedic, geriatric and general profile.The scientific novelty of the obtained results lies in the substantiation of the scientific and theoretical foundations for the creation of a practical program of physical therapy for the elderly with the consequences of total knee arthroplasty, which occur against the background of geriatric syndromes. For the first time, a comprehensive program of physical therapy for elderly people with knee endoprosthesis and geriatric syndromes was developed, the defining features of which are the performance of therapeutic exercises of various orientations and functional training using the platforms "PROSEDOS", Otago exercise program, manual influence (massage, post-isometric relaxation), correction nutrition, patient education, which distinguishes it from generally accepted programs of rehabilitation and physical therapy; received data on the peculiarities of the course of geriatric syndromes in elderly patients with knee endoprosthesis (in addition to the geriatric status, the functional state of the operated limb and their general impact on the health of the examined persons); a comprehensive program of physical therapy for the elderly with the consequences of total knee arthroplasty, occurring against the background of geriatric syndromes, is built in accordance with the International Classification of Functioning, Disability and Health, taking into account its main components and the characteristics of each condition, and the ultimate goal of restorative measures is aimed at on the level of participation in life situations and / or professional activities.
